These establishments received demerits for being out of compliance as stated:

(8) 575 Pizzeria, 7710 Hillside Road: Ranch, salsa and candied jalapenos need to be labeled with ingredients, weight, manufacturer and allergens. COS. Several flying pests in kitchen. To be corrected within three days. Pizza cutting table needs to be repaired or replaced; ANSUL system needs to be inspected every six months. To be corrected within 180 days.

(5) Big Spencer, 4101 S. Loop 335 East.: Food manager needed; bathroom door needs to be self-closing; coving needs to be repaired; fire extinguisher needs to be inspected. To be corrected within 20 days.

(5) Eskimo Hut, 7200 McCormick Road: Drink with no lid in food area. COS. Side of ice cream machine missing and needs to be replaced. To be corrected within 10 days. Bucket in hand sink; ceiling tiles above soda machine, ice cream machine and three-compartment sink must be smooth, durable, non-absorbent and easily cleanable; floor in back room needs to be sealed or painted. To be corrected within 90 days.

(4) Freddy’s Frozen Custard & Steak, 5225 S. Coulter St.: Soda machine has air break but needs to be changed to air gap; ANSUL system needs to be inspected every six months. To be corrected within 180 days.

(12) Golden Corral No. 533, 7220 W. I-40: Dirty wares throughout kitchen on clean racks (second violation); at least one service sink or one curbed cleaning facility equipped with floor drain must be provided and conveniently located for cleaning of mops; employees preparing brownies (TCS food) with bare hands. To be corrected within 10 days. Non-food contact surfaces must be constructed of a corrosion-resistant, non-absorbent and smooth material; all broken, cracked and worn out tiles must be replaced (second violation); all ceiling tiles and paint on grid must be repaired or replaced; debris in sinks and dirty water being poured in hand sink. To be corrected within 30 days.

(5) Grandma’s Burritos, 7012 S. Western St.: Chips stored uncovered under prep table. To be corrected within three days. Utensils in hand sink; no soap at hand sink; light in restroom needs a light shield. To be corrected within 90 days.

(5) Heritage Convalescent Center, 1009 Clyde St.: Keys on prep line; wiping cloths must be held in chemical sanitizer at proper strength in between uses. COS. Direct drain must be indirectly plumbed for washing food products. To be corrected within 90 days.

(8) On The Border Mexican Cafe, 2401 S. Soncy Road: Salsa held at room temperature; Barbacoa in hot holding at improper temperature; hair restraints needed; eating or drinking not allowed in booth. COS.

(6) Petro Dunkin Donuts 307, 8500 E. I-40: Back door propped open; ice scoop cannot be stored in ice machine that is self-replenishing; ice bucket must be stored inverted. COS. Food establishment permit expired; door sweep still needed on back door (repeat); last inspection and current food manager cards must be posted in view of public; hat or hair nets needed around food prep or while working in kitchen (repeat).

(5) Petro Stopping Centers C S , 8500 E. I-40: Hot dogs in hot holding at improper temperature; sandwich unit holding at improper temperature. COS.

(1) Petro Stopping Centers I S R, 8500 E. I-40: Ceiling tiles in mop room need to be smooth, non-absorbent and easy cleanable. To be corrected within 180 days.

(3) Petro Stopping Centers T S, 8500 E. I-40: Soda nozzles needed to be cleaned. COS. Food manager not registered. To be corrected within 180 days.

(44) Poblano’s Grill, 900 S. Tyler St.: Chicken in hot holding at improper temperature; no date marking used on food items; sanitizer solution at improper strength; pesticide on prep line; air gap needed on food prep sink; shrimp in cold holding at improper temperature; food containers sitting on top of raw food; pork mixed with beef products; raw food sitting above cooked food; eggs above food products; adulterated food in flour (signs of rodent activity); drain line capped with a rag. To be corrected within 24 hours. Shell Stock tags are required to be attached until container is empty or retagged and thereafter kept on file for 90 days; cold holding equipment not holding at proper temperature; dirty plates; non-food contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris; improper lighting throughout kitchen and walk-in; all employees need food handler cards; food manager card needed and must be posted; all exit doors and bathrooms must be self-closing and tight fitting; lid on female refuse container needed; keys on prep area; holes throughout facility on the outside walls; blueprints needed of additions; wet rags not stored properly; sanitizing solution must be stored off of floor; knives, forks and spoons that are not pre-wrapped must be presented so that only the handles are touched by employees and by consumers if consumer self-service is provided; major cleaning needed throughout facility; smooth, durable and easily cleanable for areas where food establishment operations are conducted; cups sitting on towel; consumer advisory needed for consumption of raw animal foods. To be corrected within 10 days. This is a failed inspection.

(3) Southwest Pool Concession, 4800 Bell St.: Commercial fridge needed by next season; screen indoor needs to be replaced. To be corrected within 365 days.

(6) Toot’n Totum No. 78, 3401 S. Soncy Road: Cooler holding at improper temperature. COS. Fridge door does not shut completely and needs to be repaired; ceiling tiles above coffee station needs to be smooth, non-absorbent and easily cleanable.

(4) Toot’n Totum No. 82, 6900 S. Coulter St.: Soda nozzles need to be cleaned daily. To be corrected within three days. Ceiling tiles above coffee station need to be smooth, easily cleanable and non-absorbent. To be corrected within 180 days.

(2) Toot’n Totum No. 97, 950 S. Buchanan St.: Ceiling vents and cold hold vents need to be cleaned; hot dog drawer needs to be cleaned. To be corrected within 30 days. Men’s restroom needs self-closing door. To be corrected within 180 days.

(5) Wal-Mart No. 7349 Fuel Center, 7255 Arden Road: Certified food manager needed. To be corrected within 30 days. Thermometer needed in dairy cooler; bathroom door needs to be self-closing. To be corrected within 180 days.
