These establishments received demerits for being out of compliance as stated:

(4) Air U Amarillo, 5461 McKenna Square.: Hand wash signs needed in men’s restrooms; food establishment permit must be posted in public view. COS. Several chemical bottles unlabeled. To be corrected within 180 days.

(3) Bizzy Bees Childcare & Preschool, 3855 Business Park Drive: Food manager card expired. To be corrected within 30 days. No vent hood over kitchen stove. To be corrected within 90 days.

(5) Buckle’s Lounge, 6800 E. I-40: Expired products on shelf past seven days. COS. Food manager needed. To be corrected within 60 days.

(9) Cactus Bar, 4831 S. Western St.: Chemical bottles not labeled. COS. No current food manager card or certificate; all employees without a food manager card must have a food handler card; walk-in cooler walls not clean; rust on shelves in cooler; food manager certificate, last inspection and permit must be posted in public view. To be corrected within 30 days.

(19) Chicken Express, 2106 Second Ave., Canyon: Bird feather in vents above cooking grease needs to be removed and area needs to be cleaned and sanitized. To be corrected within 24 hours. Clean rags stored near/above food product; glass cleaner stored above single serve containers; items in walk-in cooler need to be covered during storage; tea nozzles and container used to catch tea covered in scale build-up. To be corrected within three days. No thermometers in kitchen; walk-in door gasket separating from unit. To be corrected within 10 days. Water damage to ceiling tile above mop sink; cup used as scoop in batter; ice build-up in walk-in cooler; personal drinks in kitchen without lids and straws; brighter lights needed in both walk-in coolers. To be corrected within 90 days.

(1) Dollar General Store No. 11828, 7040 McCormick Road: Ceiling tiles in restroom need to be smooth, durable, easily cleanable and non-absorbent. To be corrected within 90 days.

(28) El Giro, 1800 Bell St.: Items on floor in walk-in freezer. COS. Employee touched spice with bare hands; salt stored uncovered and handle touching bulk container; prep sink must have air gap (repeat violation and a re-inspection fee is charged); multiple toxic substances in kitchen without proper identification; utensils touching bulk container and bulk product; light shield missing in dry storage outside building; door to separate break room and bathroom to kitchen must be self-closing. To be corrected within 24 hours. No consumer advisory warning in facility regarding shrimp and shellfish on menu; exterior doors must be tight fitting to eliminate pest infestation; gaskets on reach-in cooler by grill are torn and need to be replaced; water bottle in prep area; wet wiping cloths must be stored in sanitizer solution; wiping cloths must not have heavy build-up; sanitizer water should not be cloudy; several items on floor throughout facility; intake and return covers must remain clean; ceiling fan blades must be free from dust. Shelves in walk-in cooler are rusting; three-compartment sink must have indirect connection; condensation leak in walk-in and dripping on food products; floor by fryer needs to be cleaned; fan and vents need to be dusted; grease build-up by grill needs to be removed; broken floor tiles need to be replaced; ventilation system needs to be inspected; food manager certificate must be registered annually with Health Department. To be corrected within 180 days.

(3) Friendship Ranch, 1108 24th St., Canyon: Counter top and shelves are peeling and chipped and need to be repaired. To be corrected within 10 days. Floor needs to be painted and sealed. To be corrected within 90 days.

(2) Furrbies, 210 S.W. Sixth Ave.: Evidence of pests (treatment needed). To be corrected within three days. Tools need to be separated from food equipment; all coolers need to be commercial units. To be corrected within 180 days.

(8) Hampton Inn & Suites, 7777 E. I-40: Eggs and fruit in cold holding at improper temperatures. COS. Food handler cards needed for new employees. To be corrected within 45 days. All food and single service items need to be at least six inches off ground at all times; gaskets on reach-in cooler need to be replaced. To be corrected within 180 days.

(23) Happy Burrito, 908 E. Amarillo Blvd.: Frozen meat stored uncovered in freezer. To be corrected within two hours. Ice machine needs to be cleaned. To be corrected within three days. Adequate ventilation is needed; Ansul system has been red-tagged (repeat violation and this issue has been forwarded to the Fire Marshall’s office); all vents need to be dusted; inside reach-in freezer needs to be defrosted and cleaned; gaskets on reach-in cooler need to be cleaned; grease build-up behind equipment needs to be cleaned. To be corrected within 10 days. No certified food manager; all employees who handle food and/or wash dishes must have food handler certificates and certificates must be kept onsite. To be corrected within 30 days. Three-compartment sink must have indirect connection (repeat violation); date marking system is needed and must be used; no thermometer in reach-in prep coolers; exterior door must be self-closing; shelves under prep tables are rusting and must be repaired or replaced. To be corrected within 45 days. Ceiling tiles in restroom, storage/mop sink area and kitchen must be easily cleanable, non-absorbent and durable (repeat violation); floor by ice machine needs to be repaired. To be corrected within 180 days. A re-inspection fee will be charged if all repeat violations are not corrected by the given date.

(3) Henk’s BBQ, 1508 S. Grand St.: A fly screen is needed in front of ventilation fan in smoker area; inadequate water pressure at hand sink in restroom. To be corrected within 10 days.

(6) Hero Nutrition, 1911 Fourth Ave., Canyon: No paper towels at hand sink. COS. Food debris in refrigerator and freezer (need to be cleaned and sanitized); deep freezer lid gasket pulling away from lid (must be replaced or repaired). To be corrected within 10 days. Ceiling tiles in restroom need to be smooth, durable, easily cleanable and non-absorbent. To be corrected within 90 days.

(12) La Fuente, 511 S. Grand St.: Dented cans for use. COS. Vent hood needs to be inspected; all gaskets need to be replaced. To be corrected within 10 days. No food handler cards. To be corrected within 30 days. Bathroom doors need self-closers installed; three-compartment sink must have indirect connection. To be corrected within 180 days.

(13) Little Nest, 1400 S.W. 10th Ave.: Personal items need to be separated and with a sign; silverware needs to be inverted; no thermometer in kitchen; reach-in cooler needs to be cleaned; proper date marking needed on all potentially hazardous foods; expired milk. To be corrected within two hours. Sanitizer too strong (machine needs to be calibrated); all exposed wood needs to be sealed to be easily cleanable. To be corrected within 10 days. No food manager. To be corrected within 45 days.

(3) Mary & Rogers Day Care, 4220 Wolflin Ave.: Three-compartment sink must have indirect connection. To be corrected within 180 days.

(4) McAlister’s Deli No. 1400, 4104 W. I-40: Certified food manager must be registered with City of Amarillo yearly; Manager must demonstrate knowledge of food safety and hygiene, must set an example for other employees through actions (nail polish and jewelry are not to be worn); Spanish hand washing signs needed in both bathrooms; floors must be cleaned; areas hard to reach or less travelled must also be maintained clean; clean any equipment bottoms or legs with accumulated soil or build-up; hang mops head down. To be corrected within 180 days.

(9) Pak A Sak No. 14, 6320 Bell St.: Utensil container dirty, as well as utensils inside container; mold in soda gun; box of cups on floor; box of hot dog wrappers on floor; self-service tea containers missing lids; employee drink in walk-in fridge with no lid or straw; employee food in fridge needs to be kept separate from customer food for sale; several dead bees on ice cream freezer. COS. Several potentially hazardous foods in fridge not date marked; ceiling tile above mop sink needs to be smooth, non-absorbent and easily cleanable. To be corrected within 180 days.

(24) Pho Sofia, 5641 E. Amarillo Blvd.: Last inspection not posted; no label or wrong label on foods; open drink on prep table; utensil containers dirty; no sanitizer for wipe-down sanitizing; food on ice not sufficiently covered with ice; ice on bottom of foods not sufficient cooling; no paper towels at hand sink; cardboard used as shelf liner. COS. No certified food manager; no food handler cards. To be corrected within 30 days.

(3) Roasters Coffee & Tea, 3429 S. Soncy Road: Bleach containers stored on utensil rack. COS.

(2) Robert E. Lee Elementary, 119 N.E. 15th Ave.: Both walk-in coolers were down over week end and all food must be discarded. COS.

(25) Rosa’s Cafe & Tortilla Factory, 4312 S.W. 45th Ave.: Chemical bottles not labeled; chemicals not stored properly; no paper towels at hand sink; food pan in hand sink; dishes stacked wet (repeat violation); stickers on pans after washing; dented cans on shelf for use; food items on floor in walk-in cooler; cell phones on food counters; gaskets to reach-in coolers not clean; top of tortilla ice chest is broken; bottom of shelves over hot hold not clean or sanitized; can opener and holder dirty; styrofoam cups used as scoops; single service case on floor; open employee drinks in kitchen; melted utensils; broken ice paddle; utensils not inverted to dry. COS. No food handler cards; food manager card not posted (repeat violation); tray counter needs caulk to seal cracks; dust on shelf over chips; need to clean shelves in walk-in, door to walk-in, step ladder and air intakes. To be corrected within 30 days. Due to repeat violations, a reinspection fee is charged.

(15) Salvation Army, 400 S. Harrison St.: All food or single service items must be stored at least six inches off ground; gaskets on coolers need to be cleaned; splash guard needed by three-compartment sink by door to protect food; expired milk; dented cans for use; silverware holders need to be cleaned; no thermometer in reach-in cooler; no date markings on potentially hazardous foods. To be corrected within two hours. Floors in walk-in need to be repaired; kitchen floor needs to be cleaned. To be corrected within 180 days.

(4) Sonic Drive In, 7100 Bell St.: Lime squeezer plastic is peeling off. COS. Food manager certificate needs to be registered with Health Department. To be corrected within 30 days.

(16) Stop N Save, 908 E. Amarillo Blvd.: Egg rolls in hot holding at improper temperature. COS. Sanitizer must be used when cleaning food contact surfaces. To be corrected within two hours. All items not in original container must be labeled; wet wipe-down rags must be stored in sanitizer buckets at all times. To be corrected within three days. Coolers and freezers need to be cleaned and freezers defrosted; items must be covered when stored in cooler and/or freezer; food and cooking equipment must be stored at least six inches off floor; inadequate lighting in walk-in cooler. To be corrected within 10 days. No certified food manager; all employees who handle food and/or wash dishes must have food handler certificates and certificates must be kept onsite. To be corrected within 45 days. Establishment is dumping mop water outside due to no splash guard at mop sink to prevent contamination from mop water to hand sink (mop water must be dumped in properly plumbed mop sink). To be corrected within 180 days.

(5) Subway, 1701 N. 23rd St., Canyon: Wire rack above three-compartment sink is rusting and needs to be repainted or replaced; several cabinet doors are peeling and need to be repaired or replaced. To be corrected within 10 days. Employee jacket stored with clean dishes; dishes stacked wet; food boxes in walk-in cooler on ground. To be corrected within 90 days.

(4) Subway/Travel Center No. 436, 715 S. Lakeside St.: Food manager not registered. To be corrected within 10 days. Walk-in cooler floor needs to be cleaned on Pilot side; vents need to be cleaned in Subway walk-in. To be corrected within 180 days.

(9) Taco Bell No. 032695, 2000 S. Coulter St.: Sanitizer not at recommended strength. COS. Heavy food build-up in taco prep tray and needs to be cleaned and sanitized frequently (repeat violation and a reinspection fee is charged); heavy build-up of dust on most vents in facility; non-food contact surfaces around prep line need to be cleaned (repeat violation); hand washing signs must be in both English and Spanish in bathrooms. To be corrected within 10 days.

Fire extinguisher/Ansul system needs to be inspected; covered trash cans needed in women’s restroom; floors and behind equipment need to be cleaned; walk-in floors need to be cleaned; all employees who handle food and/or wash dishes must have food handler certificates and certificates must be kept onsite; a date marking system is needed and must be used; no certified food manager. To be corrected within 30 days.

(17) Walmart Supercenter No. 793, 1701 N. 23rd St., Canyon: Food sitting on ground in walk-in cooler; bread in package touching trash can next to hand sink; pan and gloves in hand sink; chemical bottles not labeled. COS. No thermometer in kitchen; push cart in back kitchen area needs to be sealed or replaced. To be corrected within 10 days. Wall in kitchen has peeling paint and area needs to be repainted and sealed; no sanitizer buckets; frozen food defrosting on top of microwave; several employee jackets hanging and making contact with clean dishes. To be corrected within 90 days.

(6) Wonder Womenz Bakery, 909 S. Madison St.: Out of date food. COS. Food manager needs to be registered with Health Department; gaskets on cooler need to be replaced. To be corrected within 180 days.

(8) YCSF Craft, 2916 Wolflin Ave.: Chemicals must not be stored above three-compartment sink or any area where can contaminate ware washing areas. COS. Fly strips must be maintained to prevent contamination; extension cords may not be used to power cooking equipment; all cooking equipment must be commercial, NSF or UL registered; light in walk-in cooler must be shatter resistant or shielded; need to clean shelving near grill, behind grill, all equipment sides and fronts where grease or soil has accumulated; change tin foil daily, if using. product removed from original container must be labeled with common name.
